Criminal Record Confirmation
Information about No Criminal Records Certificates
Dear Students,
As of February 15, 2024, a new statutory obligation has come into force for students who are to be admitted, as part of the implementation of their study program (clinical
exercises/practical classes with patients, student internships, summer clerkship), to activities related to treatment, education, or care of minors (i.e., individuals under 18 years of age).
In connection with the amendment of the Act of May 13, 2016, on the prevention of threats of sexual crime until the end of the first semester, students of the following courses: medicine in Polish and English language (1st-year students), dentistry and medicine in Polish and English language (1st-year students) are obliged to submit:
I. Certificate from the National Criminal Register (NCR) – Polish citizens and foreigners - obtained in Poland - original, paper version required
II. Declaration of the country or countries where the student has resided in the last 20 years, other than the Republic of Poland and the country of citizenship, along with
information from the criminal records of those countries obtained for the purposes of professional or voluntary activities related to contacts with children – Polish citizen
and foreigner.
III. Information from the criminal register of the country of citizenship obtained for the purposes of professional or voluntary activities related to contacts with children
along with sworn translation – foreigners, and students who resided in countries other than the Republic of Poland or the country of citizenship [Certificates in English do not require sworn translation]. Original, paper version required
If the country of citizenship does not issue criminal records certificates, students must submit a signed declaration.
